Mileage Reimbursement

If you took your own vehicle while on the clock to complete a work related errand, please submit this form to receive reimbursement. The rate of reimbursement is determined by the state of California to fairly cover the expense of gas usage, as well as, wear and tear in driving on the vehicle.
